The “Literature Orders—Simple Template” is a PDF form designed specifically for Literature Servants in English congregations. Unlike the interactive version, this simple template is primarily for printing blank forms. It provides a structured layout for tracking and organizing literature requests without digital data‑entry capabilities.

This is not an official form. This file cannot be redistributed outside of the website where it was posted. The form is currently only available in English. A separate file will be made available soon for use by congregations not in English.
This template was first produced in 2018, then revised in 2020, and again in 2025.
