Family Connections Being Lost?

We've noticed that our database seems to be losing information about family connections. The family head is checked for one publisher, but all the related family members have suddenly lost the name of the family head and the association. When I run a data integrity check it turns up bunches of them at once. We have to manually re-select the various members, otherwise the 'Family' setting in territories doesn't work (obviously). We are all updated to 8.1 for the back end and 4.0.4 for the app.

 

Has anybody else seen this happening?
