And you don't put your tag/lock on another tag/lock - you put it on in a way the power CANNOT be turned on as long as a tag/lock is in place - no hanging you tag/lock on someone else's tag/lock because there is "not enough room"

At the refinery we used lock boxes for large LOTO, it was basically a small tool box that could be locked. All of the keys for the primary locks went into the lock box, then each worker would put their personal lock on the lock box. Every one had a multiple lock hasp they could use so there was always enough room for everyones personal lock.

I remember being on one worldly building site many many years ago, the electrician had a wonderful way of cutting the power to any unit or house that he was working on; he took out the council fuse. This is the main fuse/connector that is between the actual power line into the switchboard and before the distribution. So the building had no power and you needed a specialised insulated tool to pull out or insert it, which was only available to certain qualified persons (usually the electricity supplier). He got zapped once from someone accidentally switching on the power (before LOTO was even a thing), and didn't want to zapped ever again.

 

But things have changed since then.
